Long Term Care Case Management
Our approach to handling these services
for individuals and their families involves the tenants of
case management to assess, plan, coordinate, and implement
ways to promote the greatest degree of independence, safety,
and comfort for our clients.
Long Term Care Case Management is not meant
to be home care or a provider of personal care. The client
can maintain independence in a safe environment by expert
coordination of systems to preserve financial resources, ensure
safety and comfort, and enjoy an optimal quality of life.
Alliance Impairment Care Managers work with
designated responsible representatives of individuals when
there is no family member available to ensure day-to-day needs
are met once the plan is developed. The key to successful
Long Term Care Case Management is to make sure all available
community services are coordinated to protect the finances
and safety of the client.
Our Care Managers are fully qualified to
provide Case Management services through our extensive experience,
training, credentials; they continue to improve their skills
and knowledge base by participating in professional development
programs and maintaining certification and licensing in their
profession.

Alliance Impairment Management’s Case
Managers will:
- Provide ongoing service to our clients
following an assessment of their needs and upon approval from
the client or designated representative of the plan of service.
- Maintain loyalty in providing services based on the
client’s best interest.
- Hold in trust any confidence given to us by the client;
disclosure or use of this information along with disclosure
of medical, financial, and social issues will be used only
when needed to assist in coordination of the care for the
client with medical providers and ancillary services as per
HIPPA regulations.
- We will refer our clients to services and organizations
that are believed to be appropriate and of good quality. We
will strive to ensure cooperation between all of the individuals
involved in providing service and care to our clients.
- We end service to our clients only after reasonable
notice time frames and will recommend a plan to our clients
to receive the services as needed.
- AIM Care Managers will negotiate fees for durable
medical equipment and supplies, home health services, medications,
home care, transportation, and physical therapy (in rehab
facilities, on-site, home, or other institutions), to ensure
cost savings for the client.
